Long An: New guidance on handling procedures when F0 occurs in communities and enterprises

Replacing Official Letter No. 7162/SYT-NV dated October 18, 2021, the Long An Department of Health issued an official letter 7253/SYT-NV dated October 21, 2021 on temporary guidance for handling procedures when F0 occurs in the community and enterprises.

Implementing the Conclusion No. 36-KL/BCD dated October 15, 2021 of the Head of the Steering Committee for Covid-19 prevention and control of Long An province; Official Letter No. 9603/UBND-VHXH dated October 1, 2021 of the Provincial People's Committee on implementing the Conclusion No. 29-KL/BCD dated September 23, 2021 of the Head of the Steering Committee for Covid-19 Prevention and Control of Long An province; Conclusion No. 36-KL/BCD dated October 15, 2021, The Department of Health temporarily guides the process of handling F0 in communities and enterprises, specifically as follows:

I. In the community

1. General provisions: Carry out random screening tests or periodic tests for high-risk people according to the regulations, handle the correct procedure when detecting cases with positive test results for SARS-CoV-2 by RT-PCR (F0) technique.

2. Process of handling F0 in the community

* Step 1: Temporarily isolate cases with positive results for SARS-CoV-2 tested by rapid antigen test or RT-PCR of pooled samples at the local isolation area or at home; Take a single sample by RT-PCR test for confirmation.

* Step 2: The doctor assesses the F0’s health status 

a) If F0 has an underlying medical condition, is pregnant or has signs of respiratory failure (rapid or difficult breathing or Sp02 below 96 percent): Give oxygen breath and contact the Health Center of the district, city or town or the commune, ward or townlet health station or mobile medical station (hereinafter referred to as the local health agency) where F0 resides so that the management agency can transfer F0 to the hospital with the function of receiving to treat Covid-19 in the same area, or transfer to the nearest Covid-19 field hospitals by ambulance.

b) If F0 is asymptomatic or has mild symptoms:

- If it is eligible for isolation and treatment at home: The health center of the district, town or city will assign the health station of the ward, commune, town or mobile medical station where F0 resides to take care and treat F0 at home according to regulations of the Ministry of Health.

- If it is not eligible for isolation and treatment at home: The medical station of the ward, commune, town or the mobile medical station where F0 resides will report to the health center of the district, city or town so that the Health Center reports the Subcommittee for F0 Coordination and Treatment to transfer them to a hospital with the function of receiving and treating Covid-19 in the same area, or to the nearest field hospital for Covid-19 treatment by ambulance or by vehicle specializing to transport F0.

* Step 3: Take care of F0

a) In case that F0 is isolated and treated at home: take care according to the guidance in Decision No. 4038/QD-BYT dated August 21, 2021 of the Ministry of Health on issuing “Temporary guidance on managing people infected with Covid-19 at home”.

b) In case that F0 is isolated and treated at a concentrated isolation facilities: Apply to cases F0 has no symptoms or has mild symptoms but it is not eligible for home isolation; care and treatment of underlying diseases, if any; the establishments are encouraged to implement measures to improve physical condition or apply combination of oriental-western medicine and traditional medicine remedies under the guidance of the Ministry of Health.

c) In case that F0 is isolated and treated at a field hospital or a hospital for treating Covid-19: care and treat according to the guidance of the Ministry of Health in Decision No. 4689/QD-BYT dated October 6, 2021 of the Ministry of Health on promulgating "Guidelines for diagnosing and treating Covid-19".

* Step 4: investigate epidemiology, handle outbreaks according to regulations.

II. At the enterprises

1. General provisions:

1.1. Each enterprise establishes a health unit, arranges a temporary isolation area according to the instructions in Plan No. 3222/KH-UBND dated October 4, 2021 of the Provincial People's Committee on restoring production and trade of enterprises in Long An province.

1.2. Carry out periodic screening and testing for employees according to the instructions in Official Letter No. 8228/BYT-MT dated September 30, 2021 of the Ministry of Health and Decision No. 4800/QD-BYT dated October 12, 2021 of the Ministry of Health on promulgating the temperary guidance on medical professionals for implementing Resolution No. 128/NQ-CP dated October 11, 2021 of the Government promulgating the temporary regulation "Safe adaptation, flexibly and effectively control the COVID-19 epidemic”. Enterprises themselves pay for testing costs for their employees.

1.3. Local health agencies always maintain their viewpoints and principles of coordination and create favorable conditions to help businesses stabilize production and trade.

2. Procedure for handling when detecting cases with positive test results for SARS-CoV-2 at production and trade establishments:

* Step 1: The Health Department performs quick separation and temporary isolation of F0 at the isolation area at the production facility or enterprise and immediately notify the local health agency, the Economic Zone Authority, the Department of Planning and Investment, Department of Industry and Trade according to management assignment for support:

a) For industrial parks and clusters: contact the health center of the district, town or city or the health station of the ward, commune or townlet or the mobile medical station where the enterprise is operating.

b) For other production and trade establishments: Contact the ward, commune, townlet health station or mobile medical station.

* Step 2: Within 24 hours after receiving the notification of F0 occurring from the enterprise, the local health authority will come directly to the enterprise to coordinate with the Health Unit to assess the health status of F0 and take a sample as a single RT-PCR test for confirmation (if only rapid antigen testing was done before).

a) If F0 shows signs of respiratory failure (rapid or difficult breathing or Sp02 < 96 percent), is pregnant, has an underlying medical condition or has signs and symptoms on the 2nd and 3rd level of the treatment tower, then give first aid immediately and contact the local health authority where F0 is working so that the management agency can transfer F0 to a hospital with the function of receiving and treating Covid-19 in the same area or to nearest field hospital for Covid-19 treatment.

b) If F0 has no symptoms or mild symptoms: Continue to comply with the provisions of Plan No. 3222/KH-UBND dated October 4, 2021 of the Provincial People's Committee on restoring production and trade of enterprises in Long An province.

- If it is eligible for isolation at the company, enterprise or production facility: Let F0 isolate at the isolation facility of the industrial park or cluster. The unit reports the list of F0 to the health center of the district, city or town where the enterprise is operating for monitoring and managing.

- If the isolation conditions are not met at the company, enterprise or production facility: Contact the health center of the district, town or city where the unit is operating to transfer the F0 at the isolation facility according to regulations (distributed by area: Can Giuoc district, Ben Luc district, Duc Hoa district, Kien Tuong town and other localities under the direction of the Provincial People's Committee and arranged by the local health department).

* Step 3: The Health unit coordinates with local health authorities to conduct epidemiological investigations and handle outbreaks according to regulations.

a) If F0 is detected in the same workshop/production line/production team/production area/work location: Take F0 to a temporary isolation area, suspend the production area at the location have F0 (within a radius of 2 meters or in the same enclosed space) to conduct cleaning and disinfecting (within 60 minutes), then restore production. Coordinate with local health authorities to implement remaining epidemic prevention and control measures, including epidemiological investigation, surveillance to detect new cases.

b) If F0 is detected in at least 02 production lines of the same workshop: Follow as part II, item 2, (step 3, point a) and implement epidemic prevention and control measures as prescribed, including cleaning and disinfecting, epidemiological investigation, surveillance to detect new cases. Perform antigen rapid test or RT-PCR (pooled sample of no more than 10) for all remaining employees of the workshop, if negative, operate again. The Health unit strengthens surveillance of people with suspected symptoms within 14 days to do tests and promptly isolate them.

c) If F0 is detected in at least 02 workshops, each workshop has at least 2 lines of F0 detection: Suspend the whole production facility to implement epidemic prevention and control measures as prescribed for the whole factory set of production facilities. Within 24 hours, the enterprise's Steering Committee for Epidemic Prevention and Control will coordinate with the health centers of districts, town and city to analyze and assess the epidemic situation, develop an epidemic control plan to soon bring the epidemic control to the next level a workshop or an entire production facility return to operation.

d) Regarding handling of cases where F1 is in close contact with F0 with a distance of less than 2 meters (standing next to F0 in the production line, sitting at the same dining table, sitting at the same table in the same office, etc.): medical isolation as prescribed (before being sent to concentrated isolation, it is necessary to do a rapid antigen test or RT-PCR single sample). If F1 then has a positive test result, proceed with the processing as prescribed.

e) Enterprises coordinate with local health authorities to continue tracing F2 according to regulations.

g) Unify some special cases in handling F0 and F1 as follows:

- F0 with CT > 30 (CT from 30 or more): If after 24 hours, the second PCR test result is negative or CT > 30, for home monitoring and medical supervision as prescribed (F1 of in this case, they are allowed to self-monitor their health at home and test 3 times in 7 days (D1; D3; D7) if negative, they can work normally).

- F0 with CT < 30 with enough shots of Covid-19 vaccine: If there are no symptoms and no underlying disease, isolate at home:

+ Test 3 times in 7 days (D1; D3; D7) if negative, they can work normally and self-monitor their health.

+ If after the 2nd and 3rd tests, there is CT > 30, the treatment is the same as above.

+ If after the 2nd and 3rd tests, the CT is < 30, then take care according to the instructions in the Decision No. 4038/QD-BYT dated August 21, 2021 of the Ministry of Health on the promulgation of “Temporary guidance on managing people infected with Covid-19 at home”. (F1 of these cases, self-monitoring at home and testing 3 times in 7 days (D1; D3; D7) if negative, they can work normally).

- F0 has had the 1st injection after 14 days: Handle if the above situations coincide or like the case where F0 has not been vaccinated with Covid-19 vaccine.

h) After handling F0, F1, the enterprises report the results of medical treatment to the management agency, Department of Health, People's Committee of district/town/city to monitor and assist in handling the epidemic.

Note: The enterprises have to pay for the costs of testing, isolation, treatment, so on according to the provisions of Plan No. 3222/KH-BYT dated October 4, 2021 of the Provincial People's Committee on restoring the enterprises’ production and trade in Long An province.

3. In case that the enterprises organize to take sample testing for employees before returning to work or during the production and trade process:

To comply with Resolution No. 128/NQ-CP dated October 11, 2021 of the Government on the Promulgation of Temporary Regulations "Safely adapting, flexibility, effectively controlling the Covid-19 epidemic"; Decision No. 4800/QD-BYT dated October 12, 2021 of the Ministry of Health on promulgating the medical provisional guidelines for implementing Resolution 128/NQ-CP; Decision No. 8228/BYT-MT dated September 30, 2021 of the Ministry of Health on guiding the testing of SARS-CoV-2 for production and trade establishments.

When F0 is detected, there are suitable situations according to the F0 process in the community. Enterprises are responsible for immediately reporting to the local health agency, Economic Zone Authority, Department of Planning and Investment, Department of Industry and Trade according to management assignment for support.

The above is a temporary guide to the F0 handling process in communities and enterprises.
